Short answer: Because of severe low blood sugar. The Symlin label carries a boxed warning that using it with insulin raises the risk of severe hypoglycemia, especially in type 1 diabetes, and that it shows up within 3 hours of a Symlin injection. To lower that risk, the label says to cut mealtime insulin doses, including premixed insulin, by 50% when Symlin is started.
Symlin (pramlintide) is used by people with type 1 or type 2 diabetes who already take mealtime insulin. Its boxed warning says Symlin with insulin increases the risk of severe hypoglycemia, particularly in type 1 diabetes, and that when severe hypoglycemia occurs it is seen within 3 hours after a Symlin injection.
The warning says serious injuries may happen if this occurs while driving, operating heavy machinery or doing other high-risk activities, and names appropriate patient selection, careful patient instruction and insulin dose reduction as critical ways to lower the risk.
Section 2.1 says that when Symlin is started, mealtime insulin doses, including premixed insulins, are reduced by 50% to reduce the risk of hypoglycemia. Blood sugar is then checked often, before and after meals and at bedtime, especially when starting or raising the Symlin dose.
After that first cut, the label says insulin doses are adjusted for each person based on glucose control and tolerability, and notes that nausea can change how much insulin is needed. It says a rise in mild-to-moderate lows should be treated as a warning sign of a higher risk of severe lows. Insulin and Symlin dose changes are made only as directed by a health care professional skilled in insulin use.
| Type 2 diabetes | Type 1 diabetes | |
|---|---|---|
| Mealtime insulin at start | Cut by 50% | Cut by 50% |
| Starting Symlin dose | 60 mcg before each major meal | 15 mcg before each major meal |
| Next steps | Up to 120 mcg | 30, 45, then 60 mcg |
| When to step up | No significant nausea for at least 3 days | No significant nausea for at least 3 days |
| If nausea persists | Back to 60 mcg | Back to 30 mcg; consider stopping if 30 mcg is not tolerated |
Section 2.1 also says that if Symlin is stopped for any reason, such as surgery or illness, the same starting steps, including the insulin cut, are followed when it is restarted.
Section 2.4 says Symlin is injected right before each major meal, defined as at least 250 kcal or at least 30 grams of carbohydrate, into the abdomen or thigh (the arm is not recommended because absorption varies), at a site separate from the insulin injection. Symlin and insulin are always given as separate injections and are never mixed. A missed dose is skipped, and the usual amount is taken at the next scheduled dose.
The MedlinePlus pramlintide page puts the same points for patients: the low blood sugar risk may be greater during the first 3 hours after an injection, especially in type 1 diabetes, the doctor will lower the insulin dose when pramlintide is started, and pramlintide is not mixed with insulin.
Symlin slows stomach emptying, so section 5.5 says oral medicines that need to work fast, such as pain relievers, are taken at least 1 hour before or 2 hours after it. Dose Track can log Symlin and insulin as separate doses at each meal, with blood sugar notes, which is the kind of record the label asks for in the early weeks.
