RAD-140 vs ostarine: what is known in people?

Human trials · Liver findings · FDA and WADA

Short answer: RAD-140 and ostarine are both oral SARMs, and neither is FDA approved.1 RAD-140 has been tested in people in one small trial, in 22 women with advanced breast cancer, where liver enzyme rises were the most common side effect.2 Ostarine has several phase 2 trials, including in healthy older adults.3 There is no RAD-140 data in healthy people and no head-to-head study.

How do RAD-140 and ostarine compare side by side?

From published human trials, case reports, FDA and the 2026 WADA List. Neither has a US label.

What did the RAD-140 trial find?

The first and only published human trial gave RAD-140 to 22 heavily pretreated women with metastatic breast cancer, at 50, 100 or 150 mg a day. The most common side effects were raised liver enzymes (AST 59.1%, ALT 45.5%) and raised bilirubin (27.3%); 72.7% had a grade 3 or 4 side effect of any cause. Its half-life was 44.7 hours.2

This was a cancer trial, so it says little about effects on muscle in healthy people. Those have not been studied in published human trials.2

What did the ostarine trials find?

In 120 healthy older adults, lean mass rose with dose over 12 weeks, and the 3 mg group improved on physical function vs placebo.3 In 159 people with cancer-related weight loss, median lean mass rose 1.5 kg (1 mg) and 1.0 kg (3 mg) by day 113.4 In 136 women with advanced breast cancer on 9 or 18 mg, grade 3 or 4 drug-related side effects occurred in 8% and 16%.5

What do FDA and the case reports say?

FDA warns that SARMs have been linked to liver injury and acute liver failure, heart attack and stroke risk, psychosis, infertility and testicular shrinkage.1 Published case reports describe liver injury after RAD-140 and after ostarine.67 Products sold online as SARMs often do not contain what the label says.9

Frequently asked questions

Has RAD-140 been tested in healthy people?+
Not in a published trial. The only human trial was in 22 women with metastatic breast cancer.2
What is the half-life of RAD-140?+
About 44.7 hours in the breast cancer trial, which the authors said supported once-daily dosing.2
Does RAD-140 affect the liver?+
Raised liver enzymes were the most common side effect in its trial, and there is a published case of cholestatic liver injury in a young man.26
Are RAD-140 and ostarine banned in sport?+
Yes. The 2026 WADA List bans SARMs at all times under S1.2 and names both RAD140 and enobosarm.8
Can I track RAD-140 or ostarine in Dose Track?+
Yes. Both are built in as oral compounds, with reminders and a log you can share with your doctor.
