Short answer: Both pair a DPP-4 inhibitor with metformin, are taken twice daily with meals, and carry a boxed warning for lactic acidosis.12 Jentadueto uses linagliptin, 2.5 mg per tablet; Janumet uses sitagliptin, 50 mg per tablet. Both are for adults, are not used below an eGFR of 30, and have limits from 30 to 45. Neither label reports a trial against the other.

Both list lactic acidosis, pancreatitis, low blood sugar with insulin or a sulfonylurea, allergic reactions, lower vitamin B12, severe joint pain, bullous pemphigoid and heart failure seen with two other DPP-4 inhibitors.12 The Janumet label also lists acute kidney failure reported after approval, and says people at an eGFR of 30 to under 45 need a lower sitagliptin dose than the fixed tablet has.2 Both say to stop the tablet for a time while food and fluids are restricted for surgery, without giving a number of days. These numbers come from separate trials with different people, lengths and methods, so they are not a head-to-head comparison.
