Short answer: The Atelvia label has a missed-dose rule in section 2.4: if the weekly dose is missed, take one tablet on the morning after you remember, then go back to one tablet a week on your originally chosen day. Two tablets are not taken on the same day. The Medication Guide adds: do not take 2 doses at the same time.
| Situation | What the label says |
|---|---|
| Schedule | One 35 mg delayed-release tablet once a week, in the morning right after breakfast (2.1, 2.2) |
| Missed the weekly tablet | Take one tablet on the morning after you remember (2.4) |
| Next dose | Return to one tablet a week on your originally chosen day (2.4) |
| Two in one day | Not taken: two tablets are not taken on the same day (2.4) |
| Medication Guide | Do not take 2 doses at the same time |
Section 2.4: "If the once-weekly dose is missed, instruct patients to take one tablet on the morning after they remember and return to taking one tablet once-a-week, as originally scheduled on their chosen day. Patients should not take two tablets on the same day."
Section 17 repeats it, writing "2 tablets". The Medication Guide: "If you miss your weekly Atelvia dose, take Atelvia the morning after you remember then return to your normal schedule. Do not take 2 doses at the same time."
Example: your Atelvia day is Monday and you remember on Wednesday. Under the label you would take one tablet Thursday morning, then your next one on Monday as usual.
Section 2.2 says Atelvia is taken in the morning immediately following breakfast, not when fasting, because of a higher risk of abdominal pain if taken before breakfast. It is swallowed whole with at least 4 ounces of plain water, without lying down for 30 minutes. The missed-dose rule says "the morning after"; it does not change these instructions.
Both are risedronate, but they have separate labels. For a missed weekly tablet the rules match: one tablet the morning after you remember, then the usual day, never two on the same day. The difference is timing: Actonel is taken before the first food or drink of the day, Atelvia right after breakfast.
