Short answer: The telmisartan label starting dose is 40 mg once a day, with a usual range of 40 to 80 mg once daily, taken with or without food. Most of the blood pressure effect shows within 2 weeks and the full effect by about 4 weeks. If more lowering is needed beyond 80 mg, a diuretic may be added. People with liver problems start low and go up slowly.
This is what the FDA label says. Follow your prescriber. Your prescriber may use a different dose or pace than the label's standard schedule.
What Telmisartan Tablets is approved for: Telmisartan tablets are an angiotensin II receptor blocker (ARB) approved to treat high blood pressure, alone or with other blood pressure medicines. The label says lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, mainly strokes and heart attacks (section 1.1). Telmisartan is the medicine in Micardis. Brand-name Micardis tablets are discontinued (Drugs@FDA, NDA 020850) and DailyMed has no current Micardis label, so this page uses Alembic's generic telmisartan tablets label.

Telmisartan is a tablet taken by mouth once a day, with or without food (section 2.1). The Patient Information says to take it at the same time each day.
It comes in 20 mg, 40 mg and 80 mg tablets (section 3). The tablets absorb moisture, so they are not taken out of the bottle until just before the dose (sections 11 and 16).
It may be used alone or with other blood pressure medicines (sections 1.1 and 2.1).

People on dialysis may develop low blood pressure on standing, so their blood pressure is closely monitored.
In people low on fluid or salt, this is corrected before starting, or treatment starts at a reduced dose under close supervision.
Digoxin levels are monitored when telmisartan is started, adjusted or stopped. Lithium levels are monitored during use together.
In trials, doses up to 160 mg did not appear to lower blood pressure further. After stopping, blood pressure gradually returned to baseline over several days to one week.
People are advised not to use potassium supplements or potassium-containing salt substitutes without asking the prescriber.
If a dose is missed, it is taken as soon as remembered. If it is close to the next dose, the missed dose is skipped and the next dose is taken at the regular time. The prescribing information itself gives no missed-dose rule.
In Dose Track you set up Telmisartan Tablets as a protocol with the dose and schedule your prescriber gave you. A protocol can hold dose steps, each with its own dose and length in days or weeks, so a step-up plan like the one on the label can be entered once and the app moves to the next step on its own. Reminders follow the schedule, and every dose is logged with the date and time, so you can see which step you are on and when your last dose was. The app follows what you enter; it does not choose or change a dose.
