L-Carnitine Injection (Levocarnitine): mg to mL, syringe units and half-life

Vial strengths · mg to mL · Units chart · Half-life ~17.4 hours

Short answer: L-Carnitine has a half-life of about 17.4 hours. People vary, so treat these as estimates.

What is L-Carnitine?

L-carnitine (levocarnitine) is a natural substance the body needs to carry long-chain fatty acids into mitochondria, where they are burned for energy. As an injection it is an FDA-approved prescription drug, Carnitor.

Also known as: levocarnitine, Carnitor.

Common strengths

Carnitor injection is 200 mg/mL, 1 g in a 5 mL single-dose vial with no preservative, labeled for intravenous use only. Carnitor also comes as tablets and an oral solution.

Worked example: mg to mL and syringe units

L-Carnitine is already a solution, so there is no mixing. The only math is turning an amount in mg into a volume. Here it is at 200 mg/mL for 100 mg.

  1. Volume to draw. 100 mg ÷ 200 mg/mL = 0.5 mL
  2. On a U-100 syringe. 0.5 mL × 100 = 50 units
The amount here is an example to show the math, not a recommendation. How much to take is a decision for you and your prescriber. Check the strength printed on your vial; the same amount is a different volume at a different strength.

Approval status and label use

The FDA Carnitor injection label approves it for carnitine deficiency caused by inborn errors of metabolism, and for preventing and treating carnitine deficiency in people with end-stage kidney disease on dialysis. For metabolic disorders the label dose is 50 mg/kg as a slow 2 to 3 minute injection into a vein or by infusion; for dialysis it starts at 10 to 20 mg/kg into the dialysis line after each session.

The label route is intravenous only. Injection into muscle or under the skin is not a labeled route. Because the vial has no preservative, the label says to discard any unused portion. Serious allergic reactions, including anaphylaxis, have been reported, mostly in people on dialysis, and seizures have been reported with both oral and intravenous levocarnitine.

L-Carnitine half-life and time to peak

  • Half-lifeabout 17.4 hours
  • Time to peak (Tmax)Not listed

The FDA Carnitor label says that after a 20 mg/kg injection into a vein, levels fell in two phases: a distribution half-life of about 0.6 hours and an apparent terminal half-life of 17.4 hours, not corrected for the carnitine the body makes itself. About 76% of the dose was excreted in urine within 24 hours. Injected into a vein there is no absorption phase, so we do not list a time to peak; by mouth, the label reports peak levels at 3.3 hours.

Numbers shown are taken from the source above. Half-life varies from person to person and with injection site.

How to track L-Carnitine in Dose Track

  1. Add L-Carnitine as a custom compound. Start typing its name and Dose Track fills in the details from its research library for you to check, and it sits next to everything else you take.
  2. Save the vial strength (for example 200 mg/mL). Every amount you log then shows in mL and syringe units.
  3. Log each shot. The app subtracts it from the vial, shows what is left, and warns you before you run out.
  4. Set reminders on your own schedule, whether that is daily, weekly or every few days.
  5. Your log is stored on your phone and no account is needed. Data only leaves it for features you choose, like AI answers and lab reading.

Frequently asked questions

How many mL is 100 mg of L-Carnitine?+
Divide the amount by the strength on the vial. At 200 mg/mL, 100 mg is 0.5 mL. At a different strength the volume changes, so always check the label. The amount is only an example for the math, not a recommendation.
How many units on an insulin syringe is 100 mg of L-Carnitine?+
On a U-100 insulin syringe, 1 mL is 100 units. At 200 mg/mL, 100 mg is 0.5 mL, which is 50 units. Multiply the mL by 100 to get units.
What is the half-life of L-Carnitine?+
The FDA Carnitor label says that after a 20 mg/kg injection into a vein, levels fell in two phases: a distribution half-life of about 0.6 hours and an apparent terminal half-life of 17.4 hours, not corrected for the carnitine the body makes itself. About 76% of the dose was excreted in urine within 24 hours. Injected into a vein there is no absorption phase, so we do not list a time to peak; by mouth, the label reports peak levels at 3.3 hours.
Is L-carnitine injection approved for weight loss?+
No. The FDA Carnitor injection label approves it only for carnitine deficiency from inborn errors of metabolism and for people with end-stage kidney disease on dialysis, given into a vein.
Can I track L-Carnitine in Dose Track?+
Yes. L-Carnitine is in Dose Track's research library, so when you add it as a custom compound its details are filled in for you to check. You can then log each shot next to your other compounds, save the vial strength so every amount shows in mL and syringe units, see how much is left in the vial, and get reminders. Your log is stored on your phone and no account is needed. Data only leaves it for features you choose, like AI answers and lab reading.
