Short answer: The labels do not support it. The Lomaira (phentermine) label says the safety and effectiveness of phentermine with any other weight-loss drug have not been established, so taking them together is not recommended. The Wegovy and Zepbound labels only rule out pairing with other GLP-1 medicines. One small 12-week trial added phentermine to liraglutide and found no extra weight loss that mattered clinically.
Lomaira is phentermine 8 mg. Its label approves it only as short-term (a few weeks) use on its own, alongside diet, exercise and behavior change (Indications and Usage, Warnings). It says the safety and efficacy of phentermine combined with any other weight-loss product, prescription, over-the-counter or herbal, have not been established, and that taking them together is not recommended. The same sentence covers serotonin medicines such as fluoxetine and sertraline.
The label also says doses of insulin or diabetes pills may need to come down on phentermine, which matters for people already on a GLP-1 for diabetes. See weight-loss pills and low blood sugar.
| Label | Limitation on combining |
|---|---|
| Lomaira (phentermine) | Not recommended with any other weight-loss drug, because safety and effectiveness are not established |
| Wegovy (semaglutide) | Not recommended with other semaglutide products or any other GLP-1 receptor agonist (Limitations of Use) |
| Zepbound (tirzepatide) | Not recommended with other tirzepatide products or any GLP-1 receptor agonist (Limitations of Use) |
The Wegovy and Zepbound labels do not mention phentermine by name. Qsymia is a separate approved capsule that already contains phentermine together with topiramate. See can you take two GLP-1 medicines together.
Tronieri and colleagues (2019) studied 45 adults who had already lost an average of 12.6% of their weight over a year on liraglutide 3.0 mg with intensive behavior support. They were randomized, double-blind, to add phentermine 15 mg or a placebo to liraglutide for 12 weeks.
| Group | Mean weight change | Lost 5% or more |
|---|---|---|
| Liraglutide + phentermine | -1.6% | 2 people (9.1%) |
| Liraglutide + placebo | -0.1% | 1 person (4.3%) |
The difference was not statistically significant. People on phentermine reported larger drops in hunger in the first 8 weeks. The authors concluded the combination appeared well tolerated but did not add clinically meaningful weight loss. The trial was small, short and used liraglutide, so it says little about semaglutide or tirzepatide.
Heart rate and blood pressure, other stimulants or serotonin medicines, and diabetes medicines all shape this question. Phentermine is a Schedule IV controlled substance, so it is prescribed and tracked separately. Dose Track can log both medicines with their doses and times, which gives the prescriber a clear record if a combination is being considered.
